FORMS

  1. NOTICE OF MEETING
    • Parent needs to be notified in writing when IEP is going to be held to discuss and plan for their student. Notice should be given 10 days prior to meeting. Send copy of Notice of Procedural Guidlines with each notice.
    • Parent needs to be given at least three opportunities to respond to Notice of Meeting. A copy of each of these notices needs to be filed in the brown Pupil Services Folder. It is very important to document all attempts to contact parent and arrange IEP meeting.
    • If parent does not respond to three attempts to convene meeting, the school team should proceed with IEP.
  2. CONTINUING NOTES/ ADDENDUM FORM
    • When IEP meeting is held just for the purpose of amending services, i.e. to add or drop DIS, the following needs to happen:
    • The IEP being amended must be CURRENT; it is not possible to amend a document over a year old. In that case, a new full review must be held.
    • The addendum IEP will need page 1, the signature page PLUS the addendum/continuing notes page and goal page.
    • On the addendum page, it is important to summarize any assessments, give service specifications of time, frequency, duration and discuss any other pertinent information, i.e. transportation.
    • When IEP meeting is held for the purpose of changing program placement, i.e. SDC to RSP, the following needs to happen:
    • The IEP being amended must be CURRENT.
    • The Addendum IEP will need page 1, the signature page plus an addendum page to discuss any and all issues that have resulted in this team recommendation.

  3. TRANSPORTATION INFORMATION FORM
    • New requests for transportation must be accompanied by Transportation Information Form. This form is completed by parent and teacher and submitted to both Special Education Services and to the Transportation Office.
    • New service takes about two weeks to be in place.
    • Transportation Office will not respond until SpED Services processes request.
    • New requests for AB3632 and DIS transportation also require this form.

  1. PRIOR INTERVENTIONS
    • This attachment is necessary for initial IEPs only. Discuss interventions taken prior to consideration for SpED service.

  2. BEHAVIOR SUPPORT PLAN
    • Use this attachment whenever student's behavior impedes learning of self or others. See page 6 Writing the IEP.

  3. CERTIFICATION OF SPECIFIC LEARNING DISABILITY
    • See page 4 of Writing the IEP
    • Attachment C is only necessary when IEP team does not agree with assessment results. (Team is overriding psychological recommendations)
    • Documentation will need to accompany this certification.

  4. TRANSITION SERVICES AND HIGH SCHOOL GRADUATION

    • Academic and behavior goal(s) need to be monitored by staff at least every report card period
