As Defined Under Idea
Under the Individuals with Disabilities Act (IDEA) students requiring health-related services and procedures that are necessary for a child with a disability to benefit from special education may receive such services under "related services."

How Are Students Referred To DIS-Nursing Services?
Student is referred via Intake Unit, parents, teachers, medical staff, placement staff or administration.

DIS-Nursing Intake

  1. Nursing coordinator reviews and/or collects data (medical history, current diagnosis/prognosis, medical treatment plan).
  2. Eligibility for DIS-Nursing is determined.
  3. Nursing coordinator assesses student at home or at Intake unit.

Preparation For IEP

  • Clarify student needs, as well as parental and staff concerns.
  • Determine the necessity of specialized health care services being provided during school hours.
  • Delineate who will provide the specialized health care services.
  • Determine how the provider will be trained if needed.

During IEP

  • Any time that nursing services are recommended, the Nursing Coordinator must be part of the IEP.
  • Current nursing protocols must be on record prior to providing any services.

Parental Responsibilities

  • Medical records are to be provided in a timely manner (recent records i.e. within the last six (6) months).
  • A Physician's Authorization form must be completed yearly, prior to receiving nursing services.
  • When a student is to go from tube feeling to oral feeding (when past medical records state otherwise) documentation from the student's physician must be received in writing.
  • Maintenance of clean and working equipment.
  • Clean supplies.
  • Medication must be in original containers provided by pharmacy.
  • When nurse/para arrive at student's home, all equipment, medication, and/or other necessary medical supplies must be ready to take to school. SFUSD staff is not required to pack these items.
  • Medication to be administered by the nurse is not to be mixed by the parent. The nurse is to prepare all medications.
  • The nurse is to remain focused on his/her role as healthcare provider. Please address all other needs to the teacher.

In Event Of Absentee Nurse/No Show

  • Call/page nursing consultant, DIS-Nursing Services.
  • Call DIS administrator.
  • Call Agency supervisor.
