Special Education Transition
"Transition planning is an essential step in preparing students with disabilities to assume adult roles. Transition planning should focus on students’ future goals, empowering them to create a personal vision and identifying opportunities to help them meet their current needs as they transition into postsecondary education and training, employment, and quality adult life."—Transition to Adult Living: An Information and Resource Guide
The Individuals with Disabilities Education Act of 2004 requires:
- Transition language in the IEP at age 16
- Measurable postsecondary goals based on age-appropriate assessments related to: training, education, employment, and, where appropriate, independent living skills
- A summary of performance upon school exit
18-22 year-old Mild to Moderate Program
The 18-22 Mild to Moderate Program is a specialized program appropriate for special education students who meet the eligibility requirements. To receive placement, a referral should be made by the student’s teacher/counselor at the current school of attendance.
SFUSD Special Education Transition Options
(18-22 year old students)
SFUSD is excited to announce the development of additional transition options beyond the existing Community Access & Transition (CAT). Students who are either applying for or are currently enrolled in a CAT class are eligible to apply.
14-22 year-old WorkAbility I Training Program
WorkAbility I is a training program for special education students ages 14-22 designed to promote career awareness and exploration while students complete their secondary education program.
WorkAbility I provides students with opportunities for job shadowing, paid and non-paid work experience, and ongoing support and guidance from vocational personnel.

- Work Permit if under 18 yrs old
- Placement form
- Baseline Data Profile
- Valid ID with date of birth.
- Social Security Card.
- Current school schedule.
All of these documents must be submitted with all required signatures before an application will be accepted.
