Where do I go to sign in daily?
For staff with an SFUSD email account use: Daily COVID-19 Staff Screening Form. The form can also be accessed at signin.sfusd.edu.
For contractors, visitors & volunteers:
- Set up an account with Smartsheet at www.smartsheet.com.
- Register using the New Registration Form with your work email address. (contractors) or the best email address to contact you (visitors/ volunteers) at.
- Proceed to sign to sign in with the Daily COVID-19 Staff Screening Form.

Will there be any open office hours?
Yes, there will be office hours support with COVID Response questions and reopening materials (Wednesday, 4/7 from 2-4 or Friday, 4/9 from 2-4).
Who do I contact if I have questions about Smartsheet?
Please email family.link@sfusd.edu with subject heading “Smartsheet Support” for any questions you have regarding the digital sign in process.

How often do I need to sign into Smartsheet?
Each day, prior to coming onsite every staff member, contractor, volunteer or visitor is expected to self screen using the Daily COVID-19 Staff Screening Form. If you are a contractor, volunteer or visitor who is signing in for the first time, you will need to create a Smartsheet account first then complete the New Registration Form prior to completing the Daily COVID-19 Staff Screening Form.
Note: You will only need to register this first time. Use your Smartsheet account to sign in to the Daily Screen Form moving forward.

What do I use to log into Smartsheet?
SFUSD Staff: Select “Google” under the “Log in with…” option. Enter your district-issued email and password. If you do not know your SFUSD email or password, call the Help Desk at 415-241-6476.
Contractors: Use your work email address (if you do not have an SFUSD email) to create a Smartsheet account at www.smartsheet.com, complete the Contractor/ Volunteer/ Visitor Registration Form

How do I sign in on behalf of a student?
Students will be required to answer the daily screening questions prior to entering their school. A health screener will use the New Case Submission Form to complete a physical screen onsite only if the student has been determined to Positive, Symptomatic or a Close Contact.
