STUDENTS:

EMERGENCY CARDS-All students must have an updated emergency card on file. If there are any changes regarding address, phone number(s), medical or other emergency information, please complete a new card.  New cards are available in the Main Office.  It is extremely important for this information to be as accurate as possible in case of injury or illness.  Please return your completed Emergency Card to your Registry Teacher. Any student with a new address will need to obtain a “Change of Address” form from the district. Completed forms need to be submitted to the EPC at 555 Franklin in person.  Students submitting a change of address/phone must come to the main office to update their emergency card.  Ms. Sandra Gomez

LUNCH APPLICATIONS: ALL SFUSD and transfer students – All SFUSD students who were eligible for free or reduced lunch as of June 2019 will have continued eligibility through September 2019.  To avoid uninterrupted status, a new 2019-2020 lunch application must be completed by parent/guardian either paper form or online at www.SchoolCafe.com by 9/6/2019.  Paper applications will be distributed and collected through registry and are also available in the Main Office.   Applications are available in English, Chinese, and Spanish.  All students are requested to submit a 2019/2020 application.

Please be sure to use UPPERCASE letters and BLACK/BLUE ink pen. Application must be legible and must be thoroughly completed, signed and dated by the parent/guardian.  We are striving for 100% return on the lunch applications in order to enhance our chances for additional funds. Even if families do not qualify or are not interested in the Free & Reduced Lunch Program, parents/guardians are asked to complete student’s name, check the box in Step 1, and sign & date application. If your application is filed late, you will have to pay for your lunch until approval is received from Student Nutrition Services. Application processing takes approximately 2-3 weeks. You will be notified via your registry as to whether or not you qualify.  Ms. Holly Giles
