How to refer a student to the program or schedule a tour:
SFUSD Staff please fill out the County Community School form and attach your school admin to the referral if possible. Please note if this is a truancy referral an eligible student must have three truancy notices and submission to the attendance office.
Non-SFUSD providers, students, families or guardians: please contact the Enrollment Center at (415) 241-6085. If the student is a current SFUSD participant, please contact your current admin or school counselor to make a referral by submitting a County Community School for.

The Transfer Panel approves all tours for C.A.R.E. Programs. Staff will work with families to find a time to conduct a tour.
