Get the latest permission forms and guidelines for day trip, overnight trip, bagged lunch, and many more!

In Arabic, Chinese, English, Filipino, Spanish, and Vietnamese. Print out the appropriate number of each language and send home with students at least 2 weeks before the date of the field trip, and collect at least 2 days before the trip.

Log into the Field Trip Services teacher portal to request bagged lunches and/or bus transportation for field trips.

Fill out this form any time a student is injured on school premises or off-campus for a school sponsored event, such as a field trip.
