The SFUSD Student Transition Form is a process to support positive transitions from PreK/TK to TK/Kindergarten, 5th grade to 6th grade, and from 8th grade to 9th grade. The site Coordinated Care Team (CCT) should work to identify students:

  1. Who are experiencing attendance, behavior, health, and/or academic challenges which affect their school success and are currently receiving supports and services at your school, 
  2. Who are not receiving supports, but services or supports are recommended for their success in their new school the following Fall semester, and/or 
  3. Who have had limited engagement. 

Once a student is identified, a CCT member should reach out to the parent/caregiver to partner and complete the form. This Student Transition Form Information document includes all the steps and details on how to complete the form and upload forms into Synergy.
