This is a reminder on your student's Emergency Cards. It is very important for us to keep track of emergency information and contacts. It needs to be filled out every school year. The best way for us to manage Emergency Cards is to have you submit them via ParentVue.

Physical Emergency Cards
If you still experience difficulties filling out via ParentVue, your child should bring home a physical Emergency Card today. If you have difficulty with the English Forms, below are the Chinese and Spanish version - their layouts are all the same. If you prefer the emergency card in Chinese or Spanish, please check-in at the school office.
