Flynn's Coordinated Care Team Link to this section
At Flynn, we want every student and family to feel supported, seen, and heard. Our Coordinated Care Team (CCT) brings together staff from across the school to make sure students are thriving socially, emotionally, and academically, and that families are part of the process.
The CCT meets every week to talk about how to support classrooms, students, and families. This includes checking in on student progress, reviewing support plans, and following up on anything that needs extra attention.
The team includes school administrators, our social worker, the community schools coordinator, instructional coaches and intervention staff, the special education team, family coordinators, and after-school program directors.
What does the CCT do each week?
Each week when we meet, we focus on a different area of support:
- Classroom Support: We help teachers strengthen classroom practices in academics, behavior, and social-emotional learning.
- Support for Individual Students and Families: We plan for upcoming meetings like IEPs, 504 plans, or mental health support. We check on how students are doing and adjust support if needed.
- Attendance Support: We look at patterns of absences or tardiness and work with families to remove barriers.
- Family Partnership: We support family engagement, community events, and connect families to needed resources.
How the CCT Can Help
If your child is having a hard time at school, or if something’s come up at home that is affecting their school life, we’re here to help.
Some examples of what we support:
- Conflicts between students
- A misunderstanding between a student and a teacher
- Communication challenges between parents and school staff
- Identity-related concerns
- Racial or cultural issues affecting students or families
