2. Permanent Residence Card Number, if applicable
3. If Married, Divorced, or Widowed - Date of
4. Occurrence (MM/YYYY)
5. 2019 Federal Income Tax Returns
6. 2019 W-2 Forms
7. Bank account information (amount in checking,
8. savings, cash, etc.)
9. Record of any Federal Assistance (SSI, welfare,
10. food stamps, etc.)
