Field Trip Checklist for Teachers
Start here. Make sure you complete all steps and return form to the Principal.
Field Trip Summary Form
Required for all field trips. Must be turned in with a list of students and chaperones before the trip.
Day Field Trip Permission Slips
In Arabic, Chinese, English, Filipino, Spanish, and Vietnamese. Print out the appropriate number of each language and send home with students at least 2 weeks before the date of the field trip, and collect at least 2 days before the trip.
Request buses and/or food from Student Nutrition
Links to request bag lunches (fill out the week before the trip) and to request buses (fill out as far in advance as possible) are found on this page.
Student Injury Report
Fill out this form any time a student is injured on school premises or off-campus for a school sponsored event, such as a field trip.

Illuminate is the district tool for seeing student data and generating reports, including demographic, academic, and standardized test information. Click on "Sign-in With Google" and use your district Google ID.
