This page has been created just for you, to help guide you through the enrollment process and help your child transition into 6th grade. ONLINE: Sign onto ParentVue to accept or reject your school assignment. If you need an activation code, a letter should have been mailed to your home on March 16th. If you have not received your activation code, please email enrollinschool@sfusd with your student’s full name, date of birth and a copy of parent Id. Mail must be postmarked by April 10, 2020. EPC will email you after we have received your response. PLEASE COMPLETE THE FOLLOWING ONLINE FORMS BY MAY 1ST IMMUNIZATION RECORDS DUE BY JUNE 1ST According to California Law, all incoming students must show proof of vaccination, including a TB test with a positive reading within the last 3 years. Presidio M.S. will not have access to the student's elementary school records until late August 2020. Please send a scan or photo of immunization records to Registrar, Susan Erigero by June 1, 2020. BEACON COMMUNITY SCHOOLS CONSENT FORM Presido Middle School is a Community School. Our onsite community partner is The Richmond Neighborhood Center Beacon Program. All students interact with Beacon staff throughout the school day. Parents must complete this form. ELECTIVE REQUEST FORM Band, Orchestra & Choir are full year electives and students are encouraged to participate in these programs for their 3 years at Presidio. To Participate in any of these 3 programs, students are required to begin in the 6th grade. Students who do not select a full year elective will be placed on the "Elective Wheel" where the will experience a variety of elective offerings (Art, Dance, Computer Science, Health, Choir, Japanese) for 9 weeks, changing with each trimester. Students who are enrolled in the JBBP (Japanese Bilingual Bicultural Program) Pathway will be enrolled in Japanese as their elective. All other classes will be held in English with general education peers. For more information about the JBBP program, Please review this handout. ZERO PERIOD ELECTIVES: Chorus & Dance are offered as zero period electives. Please arrive promptly at 8:10 AM in room 333 for Chorus and room 226 for Dance on Monday, August 17, 2020 (first day of school) to audition. PHYSICAL FORM Please complete if your student intends to try out for a Presidio after school athletic team. Please submit to Athletic Director Shauna Alvarez by Monday, August 17, 2020. |
