Reporting a Workplace Injury or Illness

Employees must report workplace injuries and illnesses to their supervisor immediately.

Report work-related incidents, injuries, and illnesses using the Employee Incident/Injury Report (Google Form)

Call the Nurse Helpline (415) 241-6392 to report injury or illness and be referred for medical treatment. 

Provide Guidelines for Workers' Compensation and Return to Work and Claim Form (Google Docs).

The HR Workers' Compensation Team must be contacted IMMEDIATELY if any of the following occurs by calling 415-241-6101:

• Employee sent to an emergency room via ambulance
• Inpatient hospitalization
• Loss of any body part (e.g., fingertip)
• Work fatality
