From the San Francisco Health Services Agency: 

Starting January 1, 2026, some changes are coming to Medi-Cal that will affect undocumented Californians age 19 and older.

What’s changing?

If you’re undocumented and not currently enrolled in Medi-Cal, you won’t be able to sign up for full medical coverage after January 1, 2026. But if you’re already enrolled, you can keep your coverage as long as you renew on time, regardless of your immigration status.

Already have Medi-Cal?

Make sure to renew your benefits when it’s time. This helps you stay covered.

Not enrolled yet?

Apply before January 1, 2026 to get full coverage.
