Work Permits are not issued “on demand”.

Completed applications submitted by 12:00 noon on Tuesday can be picked up in the main office between 1:00 PM and 5:00 PM the following Friday.

To request a work permit:
- Pick up a blank application (Form B1-1) in the main office, or print one yourself by following this link: STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT–CERTIFICATE OF AGE

Student, guardian(s), and employer must fill out the following sections of the application COMPLETELY before submitting the application to Lowell:
- “Minor’s Information”
- “School Information”
- “To be filled in and signed by parent or legal guardian”
- “To be filled in and signed by employer”
- Deposit your completed application in the locked box in the main office labeled “SUBMIT COMPLETED WORK PERMIT APPLICATIONS HERE”.
- If a submitted application was completed correctly, students can pick up their work permit from the main office according to the schedule above.
- Incomplete or incorrect applications will be returned with a post-it note indicating which sections need to be completed or corrected before re-submission.
