How to Apply for Early Education Programs: PreK

STEP 1: Fill Out An Application

Pick up an application at the Early Education Enrollment Office at Bayview or download a copy from this webpage. Make sure you complete all sections and sign and date the application form. You can also complete and submit an online application at www.earlylearningsf.org

 

STEP 2: Submit Your Child's Application

Applications are accepted throughout the year. You can submit your application online or by mail. If you would like to come in-person to submit an application, you must first call  (415) 401-2500 to schedule an appointment at the Bayview Early Education Enrollment Office on Wednesdays from 9:00 AM to 3:00 PM. Once you have submitted your application, you will be placed on a waiting list for the school(s) you have requested.

STEP 3: Verify Your Income

An Enrollment Representative will contact you to schedule an online interview to verify your income. Please have one of the following documents ready:

• Paycheck stubs, or other record of wages issued by your employer
• A signed letter stating the source and amount of your income
• A copy of your most recently signed and completed tax return with a statement of current estimated income for tax purposes, or other business records

If you do not have these documents, an Enrollment Representative will work with you to get another source of income verification.

 

STEP 4: Prepare Your Documents

• Parent/Guardian’s Picture ID
  • CA Driver’s License, U.S. Passport, or Green Card
• Proof of Child’s Birth
  • Birth Certificate or Hospital Record for all children under the age of 18 in the household

         If the document does not contain the parent/guardian’s name, additional guardianship verification is required.

• Health Exam & Immunization Records
  Please have your child’s primary care physician complete the Medical Exam Form (available at our Early Education Enrollment Offices). If you do not have a physician, please contact the Department of Public Health at 1-800-300-9950 for information and referral for health services.
• Proof of Residency (Home Address)
  Proof of home address that include the name and address of the parent or guardian

STEP 5: Enroll in SFUSD’S PreK

If you are unable to complete your enrollment online, when you receive a placement offer, you may schedule an enrollment interview with a specialist at an Early Education Enrollment Office. At the time of your interview, you must bring the following documents:

• Parent or Guardian’s picture ID
• Birth Certificate or Hospital Record for all children under the age of 18 in the household
• Child’s physical exam and immunization record (including Tuberculosis Skin Test within one year and Varicella, which must be up-to-date for prek children)
• Proof of home address

During the enrollment interview, you will sign a contract that allows your child to participate in a SFUSD Pre-Kindergarten.

COST & FEES

Subsidized PreK

Qualifying families may take advantage of a sliding scale, subsidized tuition based on family income and need. To see if you qualify, contact the Early Education Enrollment Office at 415-401-2500.

Tuition-based PreK

Families who do not qualify for subsidized PreK may enroll on a first-come, first-served basis at $1,850 a month. There is also a non-refundable application fee of $100, which must be submitted with your application for each child you wish to enroll.

PreK OPTIONS

Full-time PreK

These programs are open five days a week throughout the year between 7:30 a.m. and 6:00 p.m. (check individual schools for exact hours). To be eligible for full-time PreK, children must turn 3-years-old on or before December 1st of the year they wish to enroll. 

School-day PreK

These programs are open during the SFUSD academic year (August - June) for six hours each day (check individual schools for exact hours). To be eligible for school-day PreK, children must turn 3-4 -years-old on or before December 1st of the academic year they wish to enroll.

步驟 5 (非必要): 報讀三藩市聯合校區學前班 

如果你無法在網上完成報讀程序, 當收到派位通知時, 你可安排會見一位早期兒童教育入學辦事處的專責職員。在會見職員時, 你必須帶備以下文件: 

• 家長或監護人附有相片的身份證
• 家中所有18歲以下兒童的出生證明或醫院記錄
• 孩子體檢及疫苗接種記錄(對於學前班學童, 須包括最新的肺結核皮膚測試(在一年內進行)和接種水痘疫苗記錄)
• 住址證明

在會見入學辦事處職員期間, 你將簽署一份允許你的孩子參加三藩市聯合校區學前班計劃的合約。 

 

學費及費用

 

政府資助學前班

合資格的家庭可受惠於按收入而獲學費資助的學前班(按家庭收入和需要而定)。欲知你是否符合資格, 請聯絡早期兒童教育入學辦事處, 電話: 415-401-2500。

 

付費學前班

不符合政府資助學前班資格的家庭可按先到先得原則報讀付費學前班, 面授教學學費為每月$1,850. 此外, 你也必須把申請表連同$100報名費(每一位申請學童)一併遞交。報名費將恕不退還。

全日制學前班

Hemos creado esta guía para ayudarlo en el proceso de inscripción. Tenga en cuenta que todos los documentos deben ser originales.

 

PASO 1: LLENE UNA SOLICITUD

PASO 3: VERIFIQUE SUS INGRESOS

Un representante del departamento de matrículas se comunicará con usted para programar una entrevista en línea para verificar sus ingresos. Tenga a mano uno de los siguientes documentos:

❏ Talones de cheque de pago u otro registro de salarios emitido por su empleador

❏ Una carta firmada que indique la fuente y el monto de sus ingresos.

❏ Una copia de su declaración de impuestos más reciente firmada y lista, con una declaración de cuales son sus ingresos estimados actuales, para propósitos de impuestos u otros registros comerciales

Si no tiene estos documentos, un representante de inscripción trabajará con usted para obtener otra fuente de verificación de ingresos.

 

PASO 4: PREPARE SUS DOCUMENTOS

❏ Identificación con foto del padre/tutor legal

● Licencia de conducir de California, pasaporte de EE. UU., o permiso de residencia permanente [Green Card]

❏ Prueba del nacimiento del niño

● Certificado de nacimiento o registro del hospital de todos los niños menores de 18 años en el hogar

Si el documento no contiene el nombre del padre/tutor legal, se requiere una verificación adicional de la tutela.

 

❏ Exámenes de salud y registros de vacunación

Pídale al médico de cabecera de su hijo que llene el formulario del examen médico (disponible en nuestras oficinas de matrícula de educación temprana). Si no tiene un médico, comuníquese con el Departamento de Salud Pública llamando al 1-800-300-9950 para obtener información, y ser remitido para obtener servicios de salud.

❏ Comprobante de residencia (domicilio)

Comprobante de domicilio que incluya el nombre y la dirección del padre o tutor legal.

 

PASO 5 (Opcional): INSCRÍBASE EN EL PRE KÍNDER DEL SFUSD

Cuando reciba una oferta de asignación escolar, usted debe inscribir a su estudiante en la escuela asignada. Si no puede rellenar su inscripción en línea, puede programar una entrevista de inscripción con un especialista en una Oficina de Matricula de Educación Temprana. Para su entrevista, debe traer los siguientes documentos:

● Identificación con foto del padre o tutor legal

● Certificado de nacimiento o registro del hospital de todos los niños menores de 18 años en el hogar

● Examen físico del niño y registro de vacunas (incluida la prueba cutánea de tuberculosis hecha en el último año, y de la vacuna contra la varicela, que debe estar actualizada para los niños en edad preescolar)

● Comprobante de domicilio

Durante la entrevista de inscripción, usted firmará un contrato que le permite a su hijo participar en un Pre kínder del SFUSD.

 

COSTO Y TARIFAS

 

Pre kínder subsidiado

Las familias que califiquen, se pueden beneficiar de utilizar una escala de ingresos para recibir la matrícula subsidiada, que tiene en cuenta los ingresos y las necesidades familiares. Para ver si califica, comuníquese con la Oficina de Matricula de Educación Temprana llamando al 415-401-2500.

 

Pre kínder con pagos de cuotas

Las familias que no califican para Pre kínder subsidiado pueden inscribirse por orden de llegada a $ 1,850 al mes para educación.   También hay una tarifa de solicitud no reembolsable de $ 100, que debe enviarse con su solicitud por cada niño que desee inscribir.
