Why do I have to complete an Individual Service Agreement (ISA) to be placed in SFUSD?

  • The ISA outlines the roles and responsibilities of a student teacher during their placement and needs to be signed.
  • The Acceptable Use Policy Agreement outlines your responsibilities around technology access for the district and needs to be signed.

    These two forms and your proof of TB Clearance and Department of Justice Clearance needs to be signed by SFUSD’s legal department then submitted and approved by the board of education. The board meets 8 times per year.

I have already submitted my TB clearance to my credential program, do I need another TB Test?

No, you can use this form and get a medical provider to sign off on your risk.

Your negative TB Clearance information is protected information, your university can not share it with SFUSD, so you need to provide this verification with your ISA.

Why does my TB clearance need to have a medical provider signature within 60 days?

  • Incoming volunteers and staff need to show proof of medical clearance for TB within 60 days.
  • You do not need a new TB test, just a consultation with a health provider who will sign off on the Risk Assessment Form that is based on your last TB test.

If I am a current employee, what do I need to do to become a student teacher?

If you are a current employee, you only need to complete the Google Form and sign the ISA. You do not need to sign the AUP, and you do not need to redo your TB clearance or Certificate of Clearance.

This applies only to current employees. Former employees must complete the full onboarding process again.
