TB Clearance for High School Interns

​Tuberculosis (TB) is an infectious disease caused by bacteria.

When a person who is sick with TB disease in the lungs (pulmonary TB) coughs, sneezes, speaks or sings they send TB germs into the air. If others breathe in the TB germs in the air, they can become infected with TB and may get sick.

All interns hired by SFUSD must have TB clearance.  UPDATE - During school closure due to COVID-19, complete this form if you need the school nurse to conduct your risk assessment over the phone.
