The Notice of Action (NOA), Application for Services

The "Notice of Approval or Denial" is a form that is completed by the parent or guardian when applying for services or at recertification. The agency representative, which may be a site clerk or enrollment specialist, will assist the parent or guardian in completing the form.

 The Notice of Action (NOA) serves many purposes:

  • The NOA notifies the family that they have been accepted into the school, under what conditions and terms and when their child can start at the school (enrollment date).
  •  The NOA also recertifies families at the end of their contract period or when there has been a change in the family's circumstances (income change, family size change, change of address, change of employment or school, etc.). 
  • The NOA notifies families when changes are made to the service agreement including, but not limited to, an increase or decrease in parent fees, an increase or decrease in the amount of services or a disenrollment of services.
