Notification: California Healthy Kids Survey

SFUSD’s Student and Family Services Division, in cooperation with the California Department of Education, will administer the California Healthy Kids Survey (CHKS) in grades 6, 7, 9 and 11 during school year 2023-24. Survey results are shared with administrators, teachers, staff, and parents/caregivers.

Survey Content. The California Healthy Kids Survey monitors health-risk behaviors among youth and young adults — including questions related to student engagement and achievement, safety, positive development, health, and overall well being.

It is Voluntary. Your child does not have to participate in the survey. Students who participate only answer the questions they want to answer and they may stop taking it at any time. However, it is very important that your child take the survey so that we can continue to improve health education programs and support services for your child and other students in our district.

It is Confidential. No names will be recorded or attached to the online survey forms or data. The results will be made available only under strict controls to assure anonymity. Parents/caregivers of children who are randomly selected to participate in the survey will receive additional information before survey administration begins. For parents/caregivers of children in the 6th grades only, your written permission is required for your child to participate in the survey. For parents/caregivers of children in the 7th through 12th grade, please make your request in writing to your child’s teacher if you wish your child to be excluded from participating in the survey.
